2> Application for “Clearance Notification”

2. Duplicate copy of Registration Certificate of the contract trade partner.

3. Original contract for this batch of goods. Duplicate copy is acceptable if the company’s seal used.

4. Information of downstream user.

5. Verification of Import/Export Right. Any one of following files is acceptable.

3> Apply for Export Clearance Notification

2. Original contract for this batch of goods. Duplicate copy is acceptable if the company’s seal used.

3. Information form indicate source of the goods.

4. Verification of Import/Export Right. Any one of following files is acceptable. 

5. Other cases:

（1） Special requirement are applied for a number of Toxic Chemicals. Pursuant to the Prior Informed Consent (PIC) procedure, the original copy of “Response Letter” from the import country’s designed national authority (DNA) is compulsory for the application. Please translation this letter into Chinese or into English when the original file is in other language than Chinese or English.

（2） For some chemicals: Arsenic and its compound products; Sodium cyanide; and Mercury and its compound products, the information form indicate the source of goods should be applied with company’s seal.

（3） For Export of Mercury and its compound products, a “opinion letter” from the local EPB is required for application.

（4） For applications for multiple Clearance Notifications, please use one Application form for one Toxic Chemical. The detailed instruction is given in guide “Use one application to apply multiple Clearance Notifications”.

4> Variation of “Registration of Certificate” (Only for the case that the name of either the Import Company or of the Export Company’s name is changed.)

1. Variation application form

   This type of application should be submitted by the export company, i.e. the foreign company introducing Toxic Chemical to China. Please indicate specifically if the representative of registration is changed. SeeApplication form of Variation of company name on Registration Certificate here.

2. Original copy of the “Registration Certificate”

3. Verification document

    A> If the name of foreign company is changed, please submit:

    Verification issued by its national administration authority to indicate that the name of said company is changed.

    B> If the name of export company is changed, please submit:

   Verification issued by industry and commerce administration (or other competent authorities) which indicates that the name of said company is changed.

5> Apply for Increase the “Original Registration Volume” of Certificate on Import of Toxic Chemicals

    Applicants qualified to increase the “Registration Volume” of Certificate will need to submit:

1. Application report

    This application should be submitted by the export company, i.e. the foreign company introducing Toxic Chemical to China. Please indicate specifically if the representative of registration is changed. Please apply company seals or signatures on the application form.

2. Original copy of Registration Certificate

3. Custom clearance forms related to the said registration certificate

6> Application for variation of clearance notification

1. Application form (company seal used on the item of applicant name)

2. Original copy of Clearance Notification

3. If the variation of Clearance Notification is resulted from a variation of registration certificate, please send a duplicate copy of the new certificate with this application form.

Note:

1. All above mentioned application forms shall be filled in with Chinese (except those items involving foreign companies) inputted by computer and printed on A4 white paper.

2. Contact details within China are preferable as contact information.
